Dental Insurance

$50.00 deductible, max $1,000 per calendar year. One free cleaning every six months.

Basic Life Insurance

First day of the month following thirty days employment/eligibility

City pays for individual coverage only

Face amount is $20,000.  Policy includes accidental death and dismemberment coverage.

Optional Life Insurance

First day of the month following thirty days employment/eligibility

Up to 12 weeks of FMLA without pay for birth of child, placement of adopted child, to care for relative with serious health condition or for serious personal illness.  Must use accumulated sick/vacation leave before going on LWOP (Leave With Out Pay).
